- Member need to send required info for verification to cashless@aia.com.mm and after that member will receive Member Declaration Form and Pre-Admission Form (can be downloaded from www.ulinkmyanmar.com/aia)

ကနဦးအတည်ပြုအဆင့် ပြီးမြောက်သောအခါ Cashless Benefit Team မှ အာမခံ ကြိုတင်တောင်းခံလွှာများ ( Preadmission Form နှင့် Member Declaration Form ) ကို ပေးပို့မည်ဖြစ်ပါသည်။ အထက်ပါလျှောက်လွှာများကို www.ulinkmyanmar.com/aia မှလည်း ဒေါင်းလုပ်ရယူနိုင်ပါသည်။
